PSLF Waiver – Form PSLF PSLF form is an application form which you have to fill out in order to be eligible for the Public Service Credit Forgiveness Plan. The PSLF is an option that permits you to have your loan forgiven in the event of 10 years’ continuous service. When you are eligible to join this program, you must work full-time for an eligible employer and be able to make 120 timely payments.
