Utah Passes Notice Of PSLF – Form PSLF PSLF form is the form you must complete in order to receive the Public Service the Loan Repayment Program. The PSLF is an opportunity for you to have your loans forgiven after 10 years of service. Before you can be considered eligible this program, you must perform full-time work for a qualifying employer, and you must make on-time 120 payments.
