Employment Verification Form Public Service Loan Forgiveness – This PSLF form is one form which you have to complete to receive the Public Service the Loan Repayment Program. The PSLF is a program that allows to have your loans forgiven following 10 years’ service. To be eligible for this program, you must be employed full-time with a qualifying employer and make 120 on-time payments.
