Download PSLF Form – A PSLF form is an application form that you have to fill out in order to apply for the Public Service Grant Forgiveness program. The PSLF is a program that allows the forgiveness of your loans at the end of 10 years’ work. To be eligible for the program, you must work full-time for a qualifying employer and be able to make 120 timely payments.

You are eligible for PSLF If you hold the qualifying loan and can make 120 qualifying repayments on that loan. You may be able apply for PSLF with only 10 qualifying payments. However, you must apply for it for it within 10 years from the commencement of the repayment period.
You don’t have to make an application for PSLF when you begin your repayment period. You may wait until you’ve made 120 payments prior to filing for it.
