PSLF Forgiveness Form – This PSLF form is the form which you have to complete you can be eligible for the public service Loan Forgiveness Program. The PSLF is an option that permits you to forgive your debts at the end of 10 years’ work. The program is available to those who qualify. the program, you need to perform full-time work for a qualifying employer and pay 120 on-time payments.

-A 501(c)(3) non-profit organisation
-A non-profit or public health care institution
-An AmeriCorps or Peace Corps volunteer service
When do I have to file for PSLF Forgiveness Form?
You can qualify to apply for PSLF when you own a qualifying loan and make 120 qualifying installments on that loan. You may be able to apply for PSLF after having made just 10 qualifying payments, but you must file during the first 10 years after the start of the repayment period.
It is not necessary to file for PSLF at the beginning of your repayment period. You have the option of waiting until you’ve paid 120 payments prior to filing for it.
