PSLF Tracking Form – Form PSLF PSLF form is an form that you have to complete in order to receive the Public Service Credit Forgiveness Plan. The PSLF is the program that allows you to forgive your debts when you reach 10 years in service. In order to be eligible the program, you need to be employed full-time with a qualifying employer and make 120 on-time payments.
