The Public Service Loan Forgiveness Program Form – This PSLF form is the form you must fill out in order to qualify for the Public Service the Loan Repayment Program. The PSLF is an opportunity for you to get your loans repaid within 10 years after you have served. To be eligible for the program, you must work full-time for a qualifying employer and pay 120 on-time payments.
